The Central West Self Management Program offers workshops proven effective for people living with chronic conditions, including arthritis, diabetes, heart disease and lung disease.
Your recommendation is important. People are more likely to participate in Self-Management workshops and follow through if their health care provider recommends it.
Participation in a self -management workshop can help to reduce symptoms, give your patient/client the confidence to manage their condition, and improve their quality of life. Self-Management participation has been shown to enhance clinical visits, reduce hospitalizations, and emergency visits.
