This page contains documents that can be used for communication between healthcare providers who are caring for patients using a multidisciplinary team approach. Providers do not have to be a part of the same practice to use these tools. The Diabetes Care Report is suggested for use by Diabetes Education Teams, as a communication back to the Client's Primary Care Provider.
